Trauma is a leading cause of death in the United States (US). Despite an existing mental health treatment gap in trauma centers, little is known about the continuum of treatment from acute care to follow-up in community settings for US patients. This study describes the mental health provision for patients in US trauma centers and the outpatient care received in community settings within 12 months of injury. This is a descriptive, retrospective cohort study of patients with trauma. Patients with injury admitted to US trauma centers from 2003 to 2011 were included. The detailed injury mechanism, mental health diagnoses, services received, including psychiatric medications, were obtained from the National Patient Safety Foundation hospital reporting system, the National Violent Injury Statistics System, and the National Survey of Ambulatory Surgery Centers. In 2011, 599 of 748 (81.2%) trauma admissions were victims of violent crime and/or motor vehicle crashes and had injury severity scores > 15. Mental health services were often lacking after trauma. Twenty-six percent of study patients with severe trauma had no mental health diagnosis or treatment after admission to trauma centers, and 16% of patients with mild and moderate trauma had no mental health diagnosis after discharge from trauma centers. The mean rate of mental health services received in 12 months after discharge was highest for substance use/alcohol use/abuse (14.3%), followed by depression (10.0%), and then anxiety disorders (8.1%). The overall mental health treatment rate was 10.4%. This is the first study to examine the mental health care provided to patients with severe trauma and after discharge from trauma centers. These findings highlight opportunities for improving mental health care delivery to trauma patients.
